2023 1095B tax forms will be available on the New Mexico Medicaid web portal by March 1, 2024. Forms will not be mailed for Tax Year 2023. Recipients may save or print a PDF copy of the form by logging on to the web portal https://nmmedicaid.portal.conduent.com.

A 1095B form is proof of full Medicaid health coverage you or members of your household received for tax year 2023; A 1095B
is not required when filing your 2023 taxes. Please utilize the web portal at https://nmmedicaid.portal.conduent.com/static/recipientlogin.htm to request a reprint.
